My husband has been with this company 5 years and they are doing a INA report on him and he does not understand what it is. Neither do I. He drives a 28 foot box truck picking up medical waste. This is the 1st time we have ever heard of it.|||This site might give you some clues: http://portal.arkansas.gov/services/Page鈥?/a>
Going by what you said ("He drives a 28 foot box truck picking up medical waste. "), the site mentions this:
Commercial Driver Record Status Watch
"Companies who wish to monitor the driving record status of their employees may set up profiles and have the system notify them of a change in driving status."
Commercial Driver Alcohol and Drug Test Reporting:
"Medical Review Officer Reporting"
Here, INA stands for "Information Network of Arkansas ". The reason photocopies of things like marriage licenses, etc. can't be sent is because the seal of the Notary Publics have to be metallic (usually gold-colored) %26amp; raised (you can feel it when you run your thumb over it) to show it is legal. Photocopies do not have a raised seal. [They look as if made with a "rubber" stamp and ink pad.] If worried about sending your copy, you might go or write to the county you were married in and request another ORIGINAL copy to send in. Tell them you misplaced your copy if all else fails. Normally, however, original copies are returned to the owner. I would enclose a self-addressed (to you, of course) stamped envelope for that purpose, and ask them (INA) nicely to return it as it is the only one you have.
Your husband has a dangerous job--medical waste is considered as HAZARDOUS MATERIAL or HazMat, for short. As Wikipedia says, "Disposal of this waste is an environmental concern, as many medical wastes are classified as infectious or biohazardous and could potentially lead to the spread of infectious disease. Examples of infectious waste include blood, potentially contaminated "sharps" such as needles and scalpels, and identifiable body parts. Sharps include used needles, lancets, and other devices capable of penetrating skin. Infectious waste is often incinerated (or burned in a high-temperature furnace). Additionally, medical facilities produce a variety of waste hazardous chemicals, including radioactive materials. While such wastes are normally not infectious, they may be classified as hazardous wastes, and require proper disposal."
So, if it IS an insurance review, go along with them. It may be just to prove he is still healthy from this type of job. Does he have to wear any type of special clothing while working--like the "HazMat suit"???
PS, you can fax the papers right from your computer if you have a CAPTURE FAX BVRP, or one attached to your printer. It requires a FAX MODEM. The following sites will show you how to do it:

[To cainvest1: INS is the Immigration %26amp; Naturalization Service that checks up on illegial immigrants].|||Probably running his name and social security number against an INA list to check for immigration status. If he is a US citizen should be no issue.|||it might be something about his insurance with the company,
